Please read the following before you apply:
Representatives of Employee Benefit Systems will be able to answer your questions and take applications electronically over the telephone. Most people will want to start with a temporary major medical plan, which will cover them up to 6 months. To qualify for this product, each covered individual must meet certain qualifications.
Coverage is not available under this plan until your current major medical benefits have ended. It is not available to anyone who is currently pregnant. Those with a history of heart or circulatory system disorder, stroke, diabetes, cancer, AIDS, alcoholism, or drug abuse will in most cases not be eligible for coverage. Further, there is a 5 year pre existing condition exclusion specifying that benefits will not be paid for expenses as a result of an existing condition which has been diagnosed or required treatment or medication within the past 5 years. This pre existing condition limitation applies only to the pre existing medical condition, and not other coverage.
The permanent plans require that the applicant answer multiple medical history questions. In many cases, a policy can be offered to someone with an existing medical condition, however that policy may have an exclusion for a particular condition or a higher premium cost.
While the cost of this coverage will vary by age, number of family members to be covered, and geographic location, it typically costs less than half of C.O.B.R.A. coverage.
