Weekly Reports of Fatalities, Catastrophes, and Other Events 

The weekly reports of fatalities and catastrophes (FAT/CAT Reports) are tabulations of fatalities or catastrophes reported by OSHA Area Offices and State Plan states via facsimile transmission of OSHA-36 forms, Fatality/Catastrophe Report, to the OSHA Directorate of Enforcement Programs (DEP) and the OSHA Directorate of Cooperative and State Programs (DCSP), respectively. These reports are entered into a database for generating weekly reports for Regional and National Offices.
