OSHA tracks worker fatalities and catastrophes

OSHA has posted weekly summaries of worker fatalities and catastrophes reported by OSHA area offices and state plan states.

The weekly reports of fatalities and catastrophes (FAT/CAT Reports) are tabulations of fatalities or catastrophes reported by OSHA Area Offices and State Plan states via facsimile transmission of OSHA-36 forms, Fatality/Catastrophe Report, to the OSHA Directorate of Enforcement Programs (DEP) and the OSHA Directorate of Cooperative and State Programs (DCSP), respectively.

These reports are entered into a database for generating weekly reports for Regional and National Offices.  Visit the Directorate of Enforcement Programs’ Worker Fatality Reports Web page for more details.
